Virtual Care Coordinator – Cigna Healthcare – Remote Position

  • * Care Coordination: Serve as the primary point of contact for members, assisting them with scheduling appointments, understanding benefits, and coordinating necessary medical services.
  • * Personalized Support: Work with members to create individualized care plans that align with their health goals, ensuring they receive timely and appropriate care.
  • * Health Education: Educate members about their health conditions, treatment options, and preventive care strategies, empowering them to make informed decisions about their healthcare.
  • * Collaboration with Healthcare Providers: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless communication and collaboration in managing member care.
  • * Case Management: Manage complex cases by addressing barriers to care, helping members navigate the healthcare system, and offering solutions to improve their health outcomes.
  • * Remote Patient Monitoring: Leverage telemedicine tools and remote monitoring technologies to track patient progress and assist in the management of chronic conditions.
  • * Data Entry & Documentation: Maintain accurate, up-to-date records of care plans, interactions, and outcomes to ensure continuity of care and compliance with Cigna standards.
  • Required Qualifications:
  • * High school diploma or equivalent; Bachelor degree in healthcare, nursing, or a related field is preferred.
  • * 2+ years of experience in healthcare, patient coordination, or case management.
  • * Strong knowledge of healthcare systems and insurance processes.
  • * Excellent communication and interpersonal skills, with the ability to engage with diverse members virtually.
  • * Proficiency in using virtual communication tools, electronic health records (EHR), and Microsoft Office Suite.
  • * Ability to work independently and manage multiple cases remotely.
  • * Strong organizational and time management skills.
  • Preferred Qualifications:
  • * Certification in case management (CCM) or health coaching.
  • * Previous experience working in a virtual or remote healthcare role.
  • * Familiarity with telemedicine platforms and remote patient monitoring technologies.
