Key Responsibilities

• Conduct assessments to determine the social, emotional, and financial needs of patients and their families.

• Offer emotional support and counseling to patients and families coping with end-of-life issues.
• Collaborate with healthcare teams to ensure coordinated and comprehensive care.

Required Qualifications

• A Master's degree in Social Work (MSW) from an accredited institution.
• Current state licensure as a clinical social worker.
• Experience in hospice or palliative care settings is preferred.

• Ability to work independently in a remote setting while staying connected with the team.
