About the position Human Touch Home Health is seeking a motivated Physical Therapist to join our growing team serving patients throughout Fairfax County, Loudoun County, and Prince William County, including Fairfax, Falls Church, Vienna, Reston, Herndon, Ashburn, Leesburg, Sterling, Woodbridge, Manassas, Gainesville, Haymarket, and surrounding communities. This PRN opportunity offers flexibility, independence, and the ability to focus on patient outcomes instead of productivity quotas. Home health offers time to educate, connect, and create meaningful outcomes. As a Home Health Physical Therapist, you'll work directly with patients in their homes, helping them improve mobility, restore function, prevent falls, and regain confidence in their daily lives. You'll have the flexibility to create a schedule that works for you while being supported by an experienced clinical team committed to your success.

Responsibilities

• Complete evaluations, reassessments, and discharge visits
• Develop individualized treatment plans
• Deliver skilled physical therapy services in patients' homes
• Educate patients and caregivers on mobility, safety, and recovery strategies
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation

Requirements

• Active Virginia Physical Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
